Call for Abstracts: Health Council of Canada National Symposium on Integrated Care | July 30, 2012

The Health Council of Canada is hosting a national symposium on integrated care. Advancing Integrated Health Care in Canada: Practices that Work will bring together policy makers, health system planners and administrators, health care providers, educators, researchers, and people who use health services, to engage in a national dialogue on health care integration in Canada.

In Canada and elsewhere, health care leaders are striving to improve their health care systems through integrated care – the delivery of coordinated, seamless care across a continuum of services – because it is believed to contribute to higher quality, patient-centred care; better health outcomes; improved efficiencies; and greater cost-effectiveness.

The Health Council invites abstracts on innovative health care practices and programs at the local, regional and/or provincial/territorial level, which have resulted in better integration of health care services. Do you have an innovative practice that you would like to tell us about? Share insights from the perspective of people who use health services (e.g. patients and their families, residents in long-term care settings), health care providers, health system planners and administrators, educators, and researchers.

  • All abstracts submitted will be considered for an oral presentation during the national symposium on October 10, 2012 in Toronto, Ontario.
